It is a misdemeanor:

1. (a) For any insurer, or any agent of any insurer, to issue a policy in violation of the requirements of subdivision (f) of section 381.
2. (b) For any insurance agent or broker to assist in arranging for the insurance where the policy violates such requirements.
3. (c) For any insurer to violate the provisions of section 382.
