The Judicial Council shall prescribe forms necessary to carry out the requirements of this chapter, including the following:

1. (a) The written statement of the obligor’s rights.
2. (b) The earnings assignment order for support.
3. (c) The instruction guide for obligees and obligors.
4. (d) The application forms required under Sections 5230, 5252, and 5261.
5. (e) The notice form required under Section 5252.
6. (f) Revised judgment and assignment order forms as necessary.
