A license that is not renewed within three years after its expiration may not be renewed, restored, reinstated, or reissued thereafter. A former licensee may apply for a new license if he or she satisfies all of the following requirements:

1. (a) No fact, circumstance, or condition exists that, if the license were issued, would constitute grounds for its revocation or suspension.
2. (b) Payment of the fees that would be required if he or she were applying for a license for the first time.
3. (c) Passage of the current licensure examination.
4. (d) He or she complies with the fingerprint requirements established by board regulation.
