1. (a) The individual must provide all the following information to request vocational rehabilitation services.

   1. (1) Name.
   2. (2) Date of Birth.
   3. (3) Disability.
   4. (4) Description of how the disability is a substantial impediment to employment.
   5. (5) Description of how vocational rehabilitation services will assist the individual to prepare for, secure, retain, advance in, or regain employment.
