Cal. Code Regs. tit. 9, § 3854 – Application Requirements.

(a) The applicant shall submit to the County an application package that includes:
1. (1) A completed loan assumption application as described in Section 3854.1.
2. (2) Written certification signed by the applicant's employer stating that the applicant is employed, or has made a commitment to commence employment in a Public Mental Health System position.
3. (3) A signed proposed loan assumption agreement, as described in 3854.2.
