1. (a) The name, address, and telephone number of the person or organization making the charge.
2. (b) The name, address, and telephone number (if available) of the person, organization, or company against whom the charge is made.
3. (c) A short statement of the facts allegedly constituting an unfair labor practice.
4. (d) The charging party may submit declarations, signed under penalty of perjury, in support of the charge. Such declarations shall remain confidential, except as provided in section 20274(a).
5. (e) Proof of service of the charge on the charged party pursuant to the provisions of Section 20206.
6. (f) The charge shall be signed by the Charging Party below the following statement: “I declare that I have read the above charge and the statements contained therein are true to the best of my knowledge and belief.”
