1. (a) The joint powers authority shall notify the Manager when a Public entity with an affiliate certificate to self insure changes its membership from the existing joint powers authority to

   1. (1) another joint powers authority;
   2. (2) a carrier insured plan or;
   3. (3) an independent self insured plan.

2. (b) The group administrator of each private group self insurer shall notify the Manager when any group member holding an Affiliate Certificate changes its membership from the existing group self insurer to:

   1. (1) another private group self insurer;
   2. (2) a carrier insured plan; or
   3. (3) an independent self insured plan.
3. (c) The Manager shall be notified no later than the date of such change.
