1. (a) Claims Administrator. A self-administered workers' compensation insurer, a self-administered self-insured employer, a self-administered joint powers authority, or a third-party claims administrator for an insurer, a self-insured employer, a legally-uninsured employer or a joint powers authority; or an attorney or agent of any of those entities.
2. (b) Claim Form. The official Division of Workers' Compensation DWC Form 1 Employee's Claim for Workers' Compensation Benefits, as set forth in Section 10139 of this Article.
3. (c) Employee. An employee, a person claiming to be an employee, his or her dependents, or agent.
