1. (a) The name of the employer, the employer's address and the workplace inspected;
2. (b) The nature of the violation, in specific terms with reference to the provision of the code, standard, regulation or order alleged to have been violated;
3. (c) Time allowed for correction of alleged violation;
4. (d) Rights of employees respecting the time fixed for correction of violations;
5. (e) Posting requirements
6. (f) Time within which an employer may contest a citation; and
7. (g) Such other information as the Division of Occupational Safety and Health deems appropriate for clear understanding of the form issued.
