1. (a) A notice filed pursuant to Education Code section 44242.5(b)(4) alleging sexual misconduct shall contain all of the following information:

   1. (1) name of the holder alleged to have engaged in misconduct;
   2. (2) name, age and address of each victim of the alleged misconduct;
   3. (3) a summary of all information known to the employer regarding the alleged misconduct; and
   4. (4) a summary of the action, if any, taken at the direct level by the employer in response to the complaint of sexual misconduct.
