1. (a) Type of program in which the college wishes to participate.
2. (b) Name of class or program.
3. (c) Number of student stations available for such participation in each.
4. (d) Any limitation to participation consistent with terms of the agreement.
5. (e) Tuition to be charged out-of-state students.
