1. (a) A description of the program's purpose, design, and organization framework;
2. (b) Priorities for enrollment;
3. (c) Family eligibility requirements;
4. (d) Conditions for participation;
5. (e) Reimbursement of providers which may include a limitation of 1.5 standard deviations from the mean market rate for the type of care provided as established by the most recent survey of the local resource and referral agency;
6. (f) Requirements for provider participation;
7. (g) Range of services available including limitations, if any, on payment for in-home care; and
8. (h) Parent fee collection policy and procedures including direct payment of fees to the provider, if allowable.
