1. (a) Any classroom teacher who meets the following qualifications is eligible to seek classification as a mentor teacher.

   1. (1) Holds a valid California teacher credential.
   2. (2) Has achieved permanent status or, in a district with an average daily attendance of fewer than 250 pupils, has been employed by the district as a credentialed classroom teacher for at least three consecutive school years prior to the school year for which nominations are to be made.
   3. (3) Has substantial recent classroom teaching instructional experience.
2. (b) Written applications for participation in the mentor program shall be submitted to the selection committee prior to a reasonable deadline established by the governing board.
3. (c) An application shall include written consent by the applicant to release of personnel information relating to his or her teaching experience and performance to selection committee members.
