1. (a) The procedures that will be used in the multiple draft weighing operation. This shall include the specific weighing location, the weighing device(s) to be used, and the position of vehicles relative to the load-receiving element of the scale.
2. (b) The make, model and a unique identification designation for each individual vehicle.
3. (c) The test data supporting accuracy of the proposed multiple draft weighing operation.

A written request for authorization is limited to those operations where the applicant has complete control of all the vehicles and weighing device(s). The written request shall be submitted to the Department and shall contain the following information:
