1. (a) Any ruling or order issued by the stewards shall specify:

   1. (1) the full name of the licensee or person subject to the ruling or order;
   2. (2) the date of birth of the licensee or person subject to the ruling or order, if known;
   3. (3) the rule or regulation violated; and
   4. (4) the penalty imposed.
2. (b) Any person affected by any ruling or order shall be notified.
