1. (a) A proof of service shall be attached to any documents filed or served under sections 184, 187, 193, 195, 196, 198 and 199.

2. (b) A proof of service shall include all of the following:

   1. (1) The address where the document was served;
   2. (2) the date of service;
   3. (3) the manner of service;
   4. (4) a statement that the person making service is over the age of 18 years; and
   5. (5) signature of the person making service, under penalty of perjury.
3. (c) Proof of electronic service may be in electronic form, subject to the provisions of sections 181.1 and 181.2.
