1. (a) Name and business address of the operator of the property or the pest control business;
2. (b) The signature of either the permittee, or the authorized representative, or the owner or qualified applicator license or certificate holder responsible to supervise the operations of the pest control business;
3. (c) Restricted material(s) to be used;
4. (d) Pest(s) to be controlled;
5. (e) Method of application;
6. (f) Criteria for determining need for the pesticide application and
7. (g) Name, address, and license or certificate number, expiration date, and category(ies) of the certified applicator responsible for supervising the possession or use of the restricted material(s).
