1. (a) Each restricted livestock drug licensee shall maintain, at the licensed location, a record of each sale of a restricted livestock drug by the licensee.

2. (b) The record of each sale of a restricted livestock drug shall include all of the following:

   1. (1) The established drug name or trade name, route of administration, quantity, and lot number(s) of the restricted livestock drug sold;
   2. (2) Date of sale;
   3. (3) Name, address, and telephone number of the purchaser;
   4. (4) Signature of the purchaser; and
   5. (5) Any additional information as required under Section 5010(c)(1) of the California Code of Regulations regarding retail sales of California prescription drugs.

3. (c) The record of each sale of a restricted livestock drug:

   1. (1) Shall be retained for a period of not less than three (3) years following the date of sale.
   2. (2) Is subject to audit by the Department and shall be made available to the Department upon request.
