1. (a) The registration form provided by the director shall include, but not be limited to, the name of the person, firm, or corporation, business address and telephone number. Any change in the name, business address or telephone number shall be reported to the director within 15 days of the change.
2. (b) There shall be no fee charged for registration.
3. (c) The registration form shall be completed by the out-of-state beef processor and returned to the director within 21 days of mailing by the director.
