(a) Each shipment of fresh or frozen meat, meat byproducts, horsemeat, horsemeat byproducts, poultry meat, or poultry meat byproducts for pet food imported into California must originate in an establishment under inspection of the Secretary or an approved state or local health agency and must be sealed at the point of origin under the supervision of the inspector in charge of the establishment. A permit for shipment must be secured from the Director, and each lot or shipment must be accompanied by a numbered certificate signed by the federal, state or local health agency inspector-in-charge showing:

1. 1. Name, address, and number of the establishment where material originates.
2. 2. All material was inspected and passed as fit for animal (pet) food by the United States Department of Agriculture, or an approved state meat inspection service, or an approved local health agency, and did not originate from animals that died other than by slaughter.
3. 3. The product has been denatured and the method of denaturing.
4. 4. The amount and type of product shipped.
5. 5. The product has been sealed and the seal number or numbers.
6. 6. The car or truck number of the vehicle hauling the product.
7. 7. The name and address of the pet food plant to which it is being shipped.
8. 8. The estimated date and time of arrival.
9. 9. The permit number (from the California Department of Agriculture) of the shipment.
10. 10. The dated signature of the Federal meat inspector, the state inspector, or the local health agency in charge at the plant of origin certifying to the above information as an official record.
