1. (a) The UPA shall issue a Unified Program Facility Permit (UPFP) that consolidates each permit issued in accordance with each program element under the Unified Program.

2. (b) The CUPA, in cooperation with its PA(s), if applicable, shall establish UPFP permitting procedures that address:

   1. (1) How a UPFP is issued and not issued to regulated businesses;
   2. (2) Conditions for which a UPFP may be revoked from regulated businesses, with conditions of reissuance;
   3. (3) Timelines and time limits of UPFP appeal processes;
   4. (4) Issuance and renewal cycles;
   5. (5) Efficient methods of issuing/transmitting UPFPs to regulated businesses;
   6. (6) Provisions for preliminary review of UPFP applications for completeness and accuracy, including facilitation of expeditious processing when necessary and establishment of a follow-up protocol regarding incomplete or inaccurate application information;
   7. (7) Provisions for technical review of UPFP applications for approval or denial by the responsible agency, including facilitation of expeditious processing when necessary and establishment of a follow-up protocol regarding approval or denial of a UPFP;
   8. (8) Provisions for tracking UPFP applications; and
   9. (9) Types of businesses and facilities that are issued UPFPs.
3. (c) Locally required supplemental information must adhere to the requirements in Section 15185(c).
4. (d) The UPA shall provide for a coordinated and consolidated UPFP process that provides regulated businesses a single point of local contact for obtaining information regarding UPFP requirements or the UPFP application process.

5. (e) The CUPA shall ensure the UPFP is coordinated, consolidated, and consistent, while minimizing duplicate information.

   1. (1) The UPFP may be either a single permit or multiple permits in a single package, which includes the UST operating permit, permit-by-rule, and any other permit or authorization requirements found under any local ordinance or requirement relating to the generation or handling of hazardous waste or materials.
   2. (2) UST operating permit requirements must be in accordance with 23 CCR, Sections 2712(c) and (i).
   3. (3) The UPFP does not include the permitting requirements of a local ordinance that incorporates provisions for the California Fire or Building Codes.
   4. (4) The UPFP may be modified by the UPA when appropriate.

6. (f) The UPFP shall include:

   1. (1) The applicable program element(s) and authorizations;
   2. (2) The agency responsible for issuing the UPFP;
   3. (3) The permitted facility by business name and address;
   4. (4) The permit issuance date;
   5. (5) The permit expiration date;
   6. (6) UST permit elements required pursuant to 23 CCR, Section 2712(c); and

   7. (7) An addendum used to document permit conditions and instructions for specific requirements to each permitted program element, if applicable.

      1. (A) UST operating permit conditions for issuance must be in accordance with 23 CCR, Section 2712.
