(b) The CUPA, in cooperation with its PA(s), if applicable, shall establish UPFP permitting procedures that address:
(e) The CUPA shall ensure the UPFP is coordinated, consolidated, and consistent, while minimizing duplicate information.
(f) The UPFP shall include:
(7) An addendum used to document permit conditions and instructions for specific requirements to each permitted program element, if applicable.
