1. (a) The contractor's proposed disposition of the grievance;
2. (b) The specific reasons for the proposed disposition;
3. (c) The names of the participants and the dates of any meetings held to discuss the grievance; and
4. (d) The procedure by which the resident may obtain a hearing to review the grievance.

If the contractor and the Resident are unable to settle the grievance informally, the contractor shall send to the resident a written summary of the grievance. The summary shall include:
