1. (a) After making a decision to carry out or approve a project for which a Negative Declaration has been prepared, the Lead Agency shall file a Notice of Determination (see Appendix B).

2. (b) Contents of a Notice of Determination shall include:

   1. (1) An identification of the project including name and location.
   2. (2) A brief description of the project.
   3. (3) The date on which the agency approved the project.
   4. (4) The determination of the agency that the project will not have a significant effect on the environment.
   5. (5) A statement that a Negative Declaration has been prepared pursuant to the provisions of CEQA.
   6. (6) The address where a copy of the ND may be examined.
3. (c) HCD shall file the Notice of Determination with the Secretary of Resources.
