1. (a) In order to request information on an attachment lien recorded with the department, the applicant shall submit a written request for attachment information signed under penalty of perjury which contains the following information:

   1. (1) The unit identifying information.
   2. (2) The registered owner(s) identifying information.
   3. (3) The name and address of the requester.
2. (b) The requester shall submit with the request for information as described in subsection (a), payment of the three dollar certificate fee.
3. (c) If a copy of the notification of attachment lien as described in Section 5640, the requester shall submit with the request for attachment information as described in subsection (a), payment of the photocopy fee per page as required in subsection 5660(m).
