1. (a) Completed application in duplicate on forms prescribed by the department.
2. (b) Two complete sets of plans, calculations, and test data when required.
3. (c) Three sets of quality control manuals or reference to applicable manuals which have previously been submitted and approved for prior models.
4. (d) When plans are submitted to the department the fees shall be as specified in Section 3060.
5. (e) If the applicant's manufacturing plan is out-of-state, the application shall include a statement signed by the applicant that he agrees to in-plant inspections and that he will apply for insignia for his factory-built housing pursuant to Section 3054.
