- (a) Upon the department's receipt of the proof of the initial training coursework and successful examination completion from a department-approved third-party provider, the department shall issue an initial certificate of compliance to the certificate holder's mailing address.
(b) Upon the department's receipt of the proof of the continuing training coursework and successful examination completion from a department-approved third-party provider, the department shall issue a continuing training coursework certificate of compliance to the certificate holder's mailing address.
- (1) Each certificate of compliance shall be valid for two (2) years from the issue date.
- (c) The department shall electronically notify a certificate holder at least ninety (90) calendar days in advance of the renewal date.
- (d) If any contact information for the park manager has changed, an amendment application shall be electronically submitted to the department within thirty (30) calendar days of the changes, identifying the prior and new information.
- (e) If the certificate holder obtains employment at a park other than the park identified on their initial certificate of compliance, an amendment application shall be electronically submitted to the department by the new park employing the certificate holder within thirty (30) calendar days of the certificate holder's hire date.
- (f) Within sixty (60) calendar days of a certificate of compliance expiration date, an amendment application shall not be approved by the department. The certificate holder must complete the continuing training coursework curriculum and examination as specified in this chapter.
