1. (a) The petitioner's name and address.
2. (b) A description of the stream system of which the determination of all rights to water is sought.
3. (c) A statement of the nature of the right or rights claimed by the petitioner.
4. (d) A statement of facts and conditions showing why the public interest and necessity will be served by a determination of all rights to water of the stream system.

5. (e) Petitioner's signature.

   If a petition is signed by more than one petitioner, the information required by (a) and (c) above shall be provided as to each petitioner.

A petition requesting a determination of the rights to water from a stream system, pursuant to Water Code Section 2525, shall be submitted to the board and shall contain the following:
