1. (a) A request for review by the State Water Board shall contain but not be limited to the following:

   1. (1) name and address of the appellant;
   2. (2) a copy of the Office of Tank Tester Licensing decision and the Division Chief's determination which the State Water Board is requested to review;
   3. (3) the manner in which the petitioner is aggrieved;
   4. (3) the specific action which the appellant wishes the State Water Board to take;
   5. (4) a copy of the evidence presented to the Division Chief prior to the determination.

2. (b) The appellant may make a written request for a hearing before the State Water Board for the purpose of presenting evidence not provided to the Division Chief.

   Any request to present evidence not provided to the Division Chief must include a statement as to why the evidence was not presented to the Division Chief for review.
