Recordkeeping.

(a) The owner or operator must submit all the following to the Unified Program Agency:
- (1) UST Operating Permit Application -- Facility Information;
- (2) UST Operating Permit Application -- Tank Information;
- (3) UST Monitoring Plan;
- (4) Underground storage tank monitoring site plan;
- (5) UST Certification of installation/modification;
- (6) Underground storage tank response plan;
- (7) Certification of Financial Responsibility in accordance with section 2809.1, if applicable;
- (8) Designated UST Operator Identification (designated UST operator name and International Code Council certification number, consistent with section 2630(b));
- (9) Statement of Understanding and Compliance Form;
- (10) Unauthorized release records in accordance with article 7;
- (11) Approval for a site-specific variance, if applicable; and
- (12) An agreement between the owner and operator in accordance with section 2691(a), if applicable.
- (b) An updated submittal must be made within 30 days of any change in the information specified in subdivision (a).
(c) The owner or operator must maintain the following records on site, or off site at a readily accessible location if approved by the Unified Program Agency, for no less than 36 months:
- (1) Designated UST Operator Visual Inspection Reports and attachments;
- (2) Facility Employee Training Certificates;
- (3) Spill Containment Testing Report Forms and attachments;
- (4) Overfill Prevention Equipment Testing Report Forms and attachments;
- (5) Secondary Containment Testing Report Forms and attachments;
- (6) Release Detection Equipment Testing Report Forms and attachments;
- (7) Tank integrity test reports;
- (8) Line tightness testing reports;
- (9) Temporary closure tank liquid sampling results;
- (10) Operation and maintenance records;
- (11) Visual monitoring inspection logs and records;
- (12) Impressed current system inspection logs;
- (13) Release detection alarm and response logs and records;
- (14) Sampling results; and
- (15) Underground storage tank closure soil and groundwater sample analytical results, including laboratory data sheets and analyses used in accordance with article 8.
- (d) The owner or operator must maintain cathodic protection test results on site, or off site at a readily accessible location if approved by the Unified Program Agency, for no less than 78 months.
(e) The following records must be maintained by the owner or operator on site, or off site at a readily accessible location if approved by the Unified Program Agency, for the life of the underground storage tank system:
(1) Installation records including, but not limited to:
- (A) Installation test results;
- (B) Manufacturer checklists and manuals; and
- (C) As-built drawings.
- (2) Records of repairs, including inspection reports and structural integrity certifications associated with interior lining;
- (3) Tank calibration charts; and
- (4) Unauthorized release reports.
- (f) Maintaining documents electronically, so that they are readily accessible on site through electronic means during inspections satisfies a requirement to maintain the records on site.
- (g) The Unified Program Agency may approve an owner or operator maintaining documents off site at a readily accessible location. The Unified Program Agency must document the approval and any conditions of the approval.
- (h) Documents maintained off site must be provided within 36 hours of being requested by the Unified Program Agency, the Board, a special inspector, or an independent compliance inspector.
