1. (a) The following reports shall be submitted monthly in an electronic data format to the State Board no later than the last day of the month following the period being reported:

   1. (1) A report listing all small water systems that failed during the previous month to comply with drinking water monitoring and reporting regulations of Title 22, Division 4, Chapters 15, 15.5, 17, and 17.5 of the California Code of Regulations; and

   2. (2) A compliance report containing the following information for each small water system under the jurisdiction of the local primacy agency that is in violation of Title 22, Division 4, Chapters 15, 15.5, 17, and 17.5 of the California Code of Regulations:

      1. (A) The name and water system identification number of the system;
      2. (B) A description of the type of violation and the standard violated; and
      3. (C) A description of any enforcement action taken by the local primacy agency with respect to the violation.

2. (b) The following reports shall be submitted quarterly in an electronic data format to the State Board no later than the last day of the quarter following the quarter being reported:

   1. (1) A list of domestic water supply permits for small water systems that have been issued, amended, or renewed during the reporting period. The list shall include the name and the identification number of the water system; and
   2. (2) A list of the small water systems for which a routine inspection or sanitary survey was conducted during the reporting period. The list shall indicate the name and identification number of the small water system and the type of routine inspection or sanitary survey performed.
3. (c) An updated inventory of small water systems under the jurisdiction of the local primacy agency shall be submitted annually in an electronic format to the State Board no later than August 15 of each year.
