Reassessment.
State of California
(a) Reassessment, at least quarterly, shall include:
(1) Progress achieved.
(2) Review and revision of goals and objectives.
(3) Revision or continuation of the individual plan of care.
(4) Preparation of a reauthorization request for continuing care.
