(a) Adult day health care services shall be requested in writing by a physician. The request shall include:

1. (1) Principal and significant diagnoses.
2. (2) Prognosis.
3. (3) Specific type of treatment and anticipated duration.
4. (4) Overall therapeutic goals for the participant.
5. (5) Medications and special diets.
