Cal. Code Regs. tit. 22, § 54203 – Participation.

(a) Participation by an eligible Medi-Cal beneficiary in adult day health care program shall require:
1. (1) A written request from a physician.
2. (2) A multidisciplinary team assessment.
3. (3) An agreement of participation signed by the participant or the participant's guardian or conservator.
4. (4) Approval by the Department.
