1. (a) The Department may approve plan contract amendments relating to mergers or reorganizations if, in addition to satisfying the conditions of Section 14303.1 or 14303.2, Welfare and Institutions Code as applicable, the following conditions are satisfied:

   1. (1) The surviving organization meets the requirements of Section 53124.
   2. (2) The information required by Section 53500 is submitted for the surviving organization.
