1. (a) Members desiring disenrollment shall make their requests to an authorized representative of the plan, on forms prescribed by the Department, or to the Department.
2. (b) Plans shall furnish disenrollment request forms to members immediately upon request by the member. Disenrollment request forms shall be mailed to a member if requested.
