1. (a) Acupuncture services are covered, subject to the limitations set forth below and in Section 51304(a), when provided by one of the following:

   1. (1) A physician, dentist or podiatrist.
   2. (2) An acupuncturist.
2. (b) Acupuncture services shall be limited to treatment performed to prevent, modify or alleviate the perception of severe, persistent chronic pain resulting from a generally recognized medical condition.
3. (c) Acupuncture is covered either with or without electric stimulation of the needles.
4. (d) Acupuncture is covered only when used to treat a condition for which treatment by other modalities is also covered.
