1. (a) The hearing officer shall determine the time and place of an informal level of review or formal hearing. The informal level of review or formal hearing shall be held at one of the following locations.

   1. (1) In the County of:

      1. (A) San Francisco if the provider resides within the First Appellate District.
      2. (B) Los Angeles if the provider resides within the Second or Fourth Appellate District.
      3. (C) Sacramento if the provider resides within the Third or Fifth Appellate District.

2. (b) Notwithstanding subdivision (a), the hearing officer may select:

   1. (1) A different place nearer the place where the provider resides.
   2. (2) Any place within the State agreeable to the parties.
