1. (a) The Record of Health Care Costs--Share of Cost, form MC 177S, shall be used to verify that health care costs have been obligated or paid by the beneficiary in an amount equal to the share of cost. The form shall be used for all beneficiaries who have a share of cost, except as specified in (b).

2. (b) Form MC 177S shall not be used for beneficiaries who meet both of the following conditions:

   1. (1) Are in long-term care.
   2. (2) Have a share of cost which is less than or equal to the monthly cost of care at the Medi-Cal reimbursement rate for the long-term care facility.
