1. (a) Each application for financial assistance shall be prepared and signed by the applicant or a person authorized to act for the applicant and to assume on behalf of the applicant any obligations imposed by law, by these regulations or by contract.
2. (b) Clinics applying for assistance shall submit a copy of their current license.
3. (c) Associations of clinics shall submit a copy of their final determination letter from the Franchise Tax Board.
