1. (a) Each application shall be submitted on a form to be provided by the Department. Funding shall be considered on a continuous basis without regard to fiscal year subject to the availability of funds.
2. (b) A clinic or association of clinics may receive only one grant in any 12-month period. Such grant shall not exceed $60,000.
3. (c) Each application shall specify a need for direct financial assistance or technical assistance to maintain and/or stabilize the health care operations of the clinic.
