1. (a) The applicant shall be in need of funds to maintain the current level of primary health care operations or to restore services to a previous level.
2. (b) The applicant shall demonstrate long-term prospects for financial stability.
3. (c) The applicant shall have the ability to match the grant or qualify for a waiver of the match.

4. (d) The applicant shall not use the grant funds to:

   1. (1) Duplicate any funding already received or contracted to be received from any funding source.
   2. (2) Provide for types of services which are eligible for funding under other programs and for which funds are available in the other programs.
   3. (3) Replace funds that have been lost due to failure to meet the terms of an agreement or contract.
   4. (4) Provide for services that are not included within the services authorized to be delivered by a primary care clinic and specified under the definition of primary health care services as specified herein.
