1. (a) The agency shall attempt to ascertain the following background information:

   1. (1) Name, address, date, place of birth, and tribal affiliation of the child and of any parent, grandparent, or great-grandparent with Indian heritage.
   2. (2) Names and addresses of extended family members.
   3. (3) Tribal customs regarding child placement practices.
   4. (4) Addresses where tribal governing organizations may be located.
