1. (a) For purposes of this section, a chronic health condition shall include one or more of the following conditions present at placement and of such nature as to make adoptive homes unavailable to the child without financial assistance:

   1. (1) Physical or mental disability present at birth or resulting from disease or injury.
   2. (2) Emotional disturbance.
   3. (3) History of either injury prior to adoptive placement, physical disease, or emotional disturbance which may manifest itself in some form of physical, mental, or emotional disability after completion of the adoption.
