1. (a) The assessment of the applicant shall include:

   1. (1) Obtaining the following identifying information:

      1. (A) Name, date of birth and sex
      2. (B) Current address and telephone number
      3. (C) Blood relationship to child, if any
      4. (D) Race and ethnic background information
      5. (E) Religion
      6. (F) Current employment information

      7. (G) Verification of marital status, if relevant

         1. 1. Verification of termination of all prior marriages.

   2. (2) Evaluation of:

      1. (A) General characteristics.
      2. (B) Social background.
      3. (C) Educational background.
      4. (D) Financial stability.
      5. (E) Work adjustment.
      6. (F) Adequacy of housing.
      7. (G) The full state criminal record of the applicant and the FBI criminal record from the DOJ.
   3. (3) Compliance with the requirements of the foreign agency that has been selected.
