1. (a) The agency shall obtain the following documentation for all applicants to facilitate the assessment:

   1. (1) Authorization for the release of medical and employment information on the applicant.

   2. (2) Report of medical examination on each applicant.

      1. (A) The report shall be signed by a licensed physician.
      2. (B) The report shall be completed prior to the placement of a child for adoption but not more than one year prior to the commencement of the assessment.
      3. (C) The report shall include a certificate that the individual is free from communicable tuberculosis for each adult residing in the home.
   3. (3) School reports for all school age children residing in the home of the applicant.
   4. (4) At least three letters of reference regarding the suitability of the applicant as an adoptive parent.

   5. (5) The full state and federal criminal record, from the State Department of Justice.
