2. (b) The agency shall obtain:

   1. (1) A certified copy of the child's birth certificate.
   2. (2) Complete information regarding the legal status of the child.
   3. (3) If the child is a dependent of the juvenile court, all court reports regarding the child's dependency.

   4. (4) Medical reports.

      1. (A) The information contained in the medical report shall be sufficient to enable the agency to comply with the requirements of Family Code Section 8706 as set forth at Section 35195(a)(1).
   5. (5) Dental reports, when they exist.

   6. (6) Authorizations for the release of:

      1. (A) Medical information.
      2. (B) Dental reports, when relevant.
      3. (C) Scholastic information, if relevant.
      4. (D) Psychological information, if relevant.
      5. (E) Medical, psychological and social information about birth parents.
   7. (7) Authorization for the child's medical care prior to relinquishment.
