1. (a) The relinquishment or consent form shall not be corrected or altered unless the correction or alteration is initialed by all of the following:

   1. (1) The parent who signed the form.
   2. (2) The reader or translator, if one was used.
   3. (3) The authorized representative of the agency.
   4. (4) The witnesses to the relinquishment (only in an agency adoption).
