1. (a) The permit for which the Department or local agency applied and whether the permit was issued or denied;
2. (b) The statute, ordinance, regulation or rule pursuant to which the permit may be issued;
3. (c) If the permit was issued, the conditions determined by the Department or local agency to be unreasonable and the reasons why such conditions would lead to a significant delay in a seismic retrofit project;
4. (d) A concise statement of the necessity of the project for the preservation of the public peace, health, or safety within the meaning of Article IV of the California Constitution; and
5. (e) If the permit was applied for by a local agency, the name, address, telephone number, and fax number, if any, of the local agency.

The appeal shall be typewritten and may be in letter or other written format. The appeal shall state that it is an appeal under Article 2 of Chapter 2 of Division 5 and shall include all of the following:
