(a) A completed Form DSA 600-A: Candidate Eligibility Application (11/20/20), incorporated by reference.
(b) Payment of the application fee corresponding to the candidate's eligibility category.
(c) DSA must be in receipt of the candidate eligibility application and application fee a minimum of two business days prior to the examination registration deadline.
