1. (a) Athlete Agents shall file the Athlete Agent Disclosure Statement ( ³/₉₈ ) and the Amendment to Disclosure Statement ( ³/₉₈ ) with the Secretary of State.

2. (b) The fees for filing the Athlete Agent Disclosure Statement and the Amendment to Disclosure Statement are as follows:

   1. (1) Athlete Agent Disclosure Statement -- $30.00
   2. (2) Amendment to Disclosure Statement -- $20.00
